HealthSnap, a U.S.-based provider of an integrated Virtual Care Management Platform, has raised $25 million in growth financing led by Eastward Capital Partners. The platform combines Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Principal Care Management (PCM), Remote Therapeutic Monitoring (RTM), and Advanced Primary Care Management (APCM) with AI-guided care coordination, virtual care delivery, and patented billing tools. The capital will accelerate its AI-powered virtual care management platform.

RPM Funding Surges Amid Value-Based Shift

The timing comes amid rising capital into remote monitoring: Athelas raised $200M in 2025 at a $6-7B valuation, while Cadence.care secured $100M Series C in Jul 2026. HealthSnap's approach integrates multiple billing programs on a single HITRUST-certified platform with patented eligibility reporting, addressing the complexity of managing separate point solutions for health systems.

Chronic Disease Burden Drives Platform Demand
